Dementia Connect for healthcare professionals

Dementia Connect, from Alzheimer’s Society, is a new personalised support service for people with dementia, their carers, families and friends.

The service connects people affected by dementia with free support and advice available by phone, online and face to face. From practical advice about living with dementia, to someone to talk to when things get tough.

To refer someone to Dementia Connect, complete our simple referral form. It only takes a minute or two to register, and you can start referring people straight away.

The national roll out of Dementia Connect relies on regional commissioning groups and organisations commissioning the service.
