Dementia Care Advisors are a consistent point of contact, providing confidential support whenever it is needed. The service can provide advice, information, and signposting to anyone living with dementia and their carer/family or friends. This could be advice about benefits, services provided by health and social care, longer term planning, living well with dementia and much more. The team work closely with local community, faith and voluntary services who are delivering dementia activities. They offer home visits, carer support and education, and attend local groups.
