About the service

Our dementia support service is here to give you personalised help and advice face to face, over the phone or in writing. Depending on your needs, we can offer you information and signposting or ongoing support regardless of whether you are worried about your memory, have a diagnosis of dementia or are a carer, family or friend. Our expert team will listen, answer your questions, and connect you to the support you need.

Who is this service for?

People with dementia.

Service user must be registered with a GP in ‘Telford and Wrekin’ or 'Shropshire'. Carers out of area can be supported if the person with dementia is registered with a GP in ‘Telford and Wrekin’ or 'Shropshire'.

When is this service available?

Monday, 9am–8pm; Tuesday, 9am–8pm; Wednesday, 9am–8pm; Thursday, 9am–5pm; Friday, 9am–5pm; Saturday, 10am–4pm; Sunday, 10am–4pm
