Mae ein gwasanaeth cymorth Dementia yma i roi cymorth a chyngor personol i chi dros y ffôn. Yn dibynnu ar eich anghenion, gallwn gynnig gwybodaeth a chyfeirio neu gefnogaeth barhaus i chi p'un a ydych yn poeni am eich cof, wedi cael diagnosis o ddementia neu'n ofalwr, teulu neu ffrind. Bydd ein tîm arbenigol yn gwrando, yn ateb eich cwestiynau, ac yn eich cysylltu â'r cymorth sydd ei angen arnoch.

People with Dementia and their carers, family and friends who speak Welsh.

When is this service available?

Dydd Llun i Ddydd Gwener (Heblaw gwyliau banc), 9.15am–4pm
