- Carers' Support Service - Leeds
Our support workers offer one to one support so that carers can discuss their issues and concerns and receive appropriate and confidential information and support. Home visits are available and assistance in claiming benefits. Referrals to and liaison with other agencies. - Day Support Services - Leeds
Person centred day care for people with early onset dementia aged 65 and under. Wide range of activities offered plus trips out, relaxation time and complementary therapies. Transport to and from the centre provided. - Community Support Service - Leeds
After a comprehensive assessment of individual needs a person centred plan of care is drawn up to focus on the individual's strengths and abilities and provide opportunities for interesting and stimulating activities in the home or in the local or wider community - Holiday Grant - Leeds
Grant of £100 per person to someone with dementia and their carer. Please note only one application allowed within a 12 month period. - Information Provision - Leeds
Advice and information on all topics related to dementia, on local services and on support of carers. - Provision of talks and training - Leeds
Provision of talks raising awareness of dementia and contribution to training of health and social services staff on dementia related topics. - Training for non-professional groups - Leeds

A bi- monthly support group providing informal, peer group support, talks by visiting speakers and occasionally complimentary therapies. Held at the Potterdale Day Centre, Dewsbury Road, Leeds. - Carers' Support Group - Yeadon - Leeds
A monthly support group providing peer group support and talks by visiting speakers. - Newsletter - Leeds

Carers Support Group - Dementia Adviser Service - Leeds
Provide information and a signposting service to people who are in the process of being diagnosed, have recently received a diagnosis of dementia and at various stages throughout the dementia journey. - Dementia Adviser Service - Leeds
Our Dementia Advisers offer personal and confidential support, advice and information to people with dementia throughout their journey. You can meet with a Dementia Adviser, either at Armley Grange or you can speak to them on the phone. You can discuss your information needs with your Dementia Adviser and how your needs can be met. This discussion will include information about services available locally. Your Dementia Adviser will contact you as often or as little as agreed in your personal information plan. Our Dementia Advisers are here to ensure you get the most out of life by guiding you to all the information you need not only about your illness, but also where to find the most appropriate services in your local area. - Leeds Services Information Provision
